Eligibility: The Maternal Intentions Program works with women of childbearing age
(14‐44) residing in Harlem who:
● Are in the last trimester of their pregnancy or in the postpartum period (6 months)
● Have been diagnosed with a chronic disease (pre‐diabetes, diabetes, gestational diabetes, hypertension, pre‐eclampsia, obesity) and
● Have had complications in their previous pregnancy.

NMPP’s Maternal Intentions program is funded by Merck for Mothers and seeks to reduce maternal mortality throughout Harlem. Maternal Intentions staff works individually (one‐to‐one sessions) and in group settings to help women manage their chronic conditions, adopt healthier behaviors, plan for their future, and transform their lives.
