Maternal & Infant Community Health Collaborative (MICHC)
The goal of the MICHC is to improve maternal and infant health outcomes for vulnerable and high-need women and to reduce social, racial, ethnic, and economic disparities in those outcomes.
Through implementation of evidence-based and/or best practice strategies MICHC works to improve specific maternal and infant health outcomes including preterm birth, low birth weight, infant mortality, and maternal mortality rates across the reproductive life course. MICHC ensures women and their families are:
- Enrolled in health insurance
- Have a primary care provider
- Connected to social support services>
MICHC also develops relationships/partnerships with community providers to better serve communities.
Eligibility: Females 13 to 45 years old residing in service areas.
