Moms and Babies Healthy Journey (Healthy Journey)

The goal of Moms and Babies Healthy Journey (Healthy Journey) is to improve health outcomes for mothers and their babies. The project provides support to women before pregnancy, during pregnancy, and after childbirth. Healthy Journey serves women ages 13 to 44, including pregnant women and parents who have a child or children under 18 months old.

Program services include:

  • Reproductive Life Planning
  • Advocacy, interconceptional care, and prenatal care access
  • Safe Sleep and Parenting Education
  • Case Management
  • Health Education
  • Birth Support
  • Referrals for housing, public assistance, and WIC
  • Domestic Violence Support
  • Home visits and escorts to health and social services appointments

Service Areas/Zip Codes:  Healthy Start serves the East, Central and West Harlem areas within zip codes 10026, 10027, 10029, 10030, 10035, 10037, and 10039. We also serve those who may be displaced or living in shelters.

Within Healthy Start, there are several projects to further assist the community in achieving overall health.

Thrive

Wellness NOW! Nutrition Project

This program facilitates weeks of nutrition counseling and education workshops.This includes:

  • Weekly exercise classes
  • Weigh-ins and healthy food demonstrations that assist with healthy eating
  • Documenting diet & exercise habits/goals
  • Wellness coaching and peer support group

These health and wellness interventions effectively reduce the incidence of overweight and obesity and improve future pregnancy outcomes among women.
